Please allow 1-2 full school days for our school staff to process your child[ren]'s registration to ensure that you and your child[ren] experience a smooth transition into our Mokapu `Ohana (family).
Please bring the following documents and forms to the office: Birth Certificate (Passport or Student Visa also accepted) * All children entering kindergarten must be 5 years of age by July 31st, of the current year. Address Verification * Military Housing Agreement Form/Lease or Utility Bill (Electricity, Landline phone, Gas, Water) Tuberculosis Clearance (PPD/Mantoux) * -If you are currently in the State of Hawai`i, your child will go through a screening process with their physician. Please visit State of Hawai`i's Department of Health page for more information regarding new process. -If you are currently out of the State of Hawai`i, the TB test must include the following information: A. Performed within 12 months of entry into a HIDOE school B. Date TB administered C. Date TB read D. Measurement of result in mm Physical Examination & Immunizations * Please contact Kaneohe Bay Health Center to make an appointment. A. Performed within 12 months of entry into HIDOE school B. Completed on a DOE Student Health Record (Form 14) Record of completed immunizations for age, including Varicella, MMR, Hepatitis B, DTaP, Polio, Hib. Documents from previous school These can include a release packet with an unofficial transcript or latest report card and for special needs students, the Individual Education Plan (IEP). Legal Documents Documents may include: Power of Attorney (if the child is not living with parents), Temporary Restraining Order, Guardianship Documents, Legal Name Change, or Court Orders. **All REQUIRED documents and completed forms are due at the time of registration.
